Did you know? When you’re discharged from the hospital, it’s usually the hospital’s responsibility to provide a 14-day supply of medication. This allows time for your ongoing prescriptions to be arranged by your GP.
However, we’ve noticed that this isn’t always happening, causing unnecessary pressure on GP services and, more importantly, confusion for patients.
To help address this we are arranging a meeting with the Chief Pharmacist at our local hospital to discuss these concerns and find a way forward. This is to ensure a smoother transition from hospital to home for our patients.
