School Lane SurgeryThe NHS COVID Pass service will close on Monday 4 December.
There are no longer any domestic requirements to demonstrate vaccination status, and no countries currently require proof of COVID-19 vaccination as a condition of entry. As such NHS England will be closing the service.
Most patients can see this information in their GP health record in the NHS App although, as mentioned above, it is no longer required.
The NHS App continues to provide a simple and secure way for you to access a range of NHS services.
