We’ve introduced a new system for scheduling Long-Term Condition (LTC) and medication reviews! You’ll now be invited for your annual review during your birthday month. This has several benefits:

- Streamlined Care: By scheduling reviews around birthdays, we can ensure a smoother flow and minimise appointment backlogs throughout the year.
- More Efficient Use of Resources: This system helps us allocate staff and resources more effectively to meet patient needs.
What is a long-term condition?
A long-term condition is a health issue that requires ongoing management, such as diabetes, asthma, high blood pressure, or depression.
Will I still receive an annual review?
Absolutely! This new system simply ensures you know when to expect your invitation.
How will I be contacted?
You’ll receive a text message (SMS) close to your birthday, inviting you to book an appointment for your LTC review. If you haven’t consented for us to send you an SMS, then we will call you instead.
I have received a link to a questionnaire. What is this for?
In some cases, we may send you a SMS with a link to a secure questionnaire we ask you to complete. This provides information and may mean that the review can be done without having to come into the surgery.
Of course, if you do wish to still come in for a review, that’s perfectly fine. We just know that for some stable, chronic conditions, patients would rather not come into the surgery if it can be avoided.
What if I have multiple long-term conditions or medications or both?
We’ll always try to combine your reviews into a single appointment whenever possible, minimising the number of visits you need to make to the surgery.
What if my birthday has already passed?
Don’t worry! We’re transitioning to the new system gradually. If you haven’t been contacted yet and feel you’re due for a review, please contact the surgery to schedule an appointment.
What if my condition or medication requires more frequent reviews?
We understand that some conditions may require more frequent monitoring. We will discuss your specific needs and schedule additional appointments as needed.
Can I choose a different month for my review?
We understand that sometimes your birthday month may not be convenient. If this is the case, please contact the surgery, and we will do our best to accommodate your preferred timing.
What if I need to see someone before my birthday month?
Not a problem! If you have any concerns or symptoms that need immediate attention, please do not wait until your birthday month. Contact the surgery to book an appointment as soon as possible.
