This form should only be completed by healthcare professionals on behalf of patients, who are registered at School Lane Surgery.

Any healthcare professional can use the form to ask the practice any prescription related questions, for example:

– Supply issues with specific medication.

– Requesting medication or appliances, after reviewing patient e.g. catheter supplies from district nurses.

– Clinical prescription queries such as for drug interactions, more cost effective options, contra-indications etc.

Please be aware that any replies from the surgery may appear in your junk or spam inbox.

I confirm that I am aware that although this form is prioritised, it may still take up to 48 hours for me to receive a reply. For very urgent matters, it may be best for me to ring and ask to speak to the Pharmacy Technician or Clinical Pharmacists.
