Vacancy – Practice Clinical Pharmacist

We are looking for a new clinical pharmacist to join the team!

For the avoidance of doubt, this is a surgery-based role. Whilst School Lane Surgery has a Pharmacy within the building, this is operated by another company, so it is not related to this role.

About the Role

At School Lane Surgery, our Practice Clinical Pharmacists plays a central role in how we care for patients. This is not a peripheral position. You will be at the heart of the practice, combining direct patient care with leadership in medicines optimisation and quality improvement.

You will take an active role in chronic disease management, lead clinical medication reviews and help patients gain confidence in their treatment. The work is varied and sometimes fast paced, but it is deeply rewarding. We work side by side, share challenges and celebrate successes together. Main Duties of the Role
You will help patients achieve the best possible outcomes from their medicines by:
• Switching medications in line with agreed and approved protocols
• Improving repeat prescribing processes within General Practice, including promoting repeat dispensing and online ordering
• Minimising clinical risk and reducing medicines waste
• Working in a patient facing capacity as part of a multidisciplinary team, you will clinically assess and treat patients using your extensive experience and in-depth knowledge. • You will take direct responsibility for the care and prescribing of patients.
